Job Description



We are seeking a dedicated Patient Care Coordinator to join our healthcare team in Harrison. In this role, you will serve as the vital link between patients, healthcare providers, and support services, ensuring seamless coordination of patient care and optimal healthcare delivery.

  • Coordinate patient care activities and manage patient flow throughout the healthcare system
  • Schedule and track patient appointments, procedures, and follow-up visits
  • Facilitate communication between patients, families, and healthcare providers
  • Maintain accurate patient records and documentation in Electronic Health Record systems
  • Monitor and update patient care plans in collaboration with healthcare teams
  • Verify insurance coverage and obtain necessary authorizations
  • Address patient inquiries and concerns promptly and professionally
  • Coordinate with various departments to ensure efficient patient care delivery
  • Track and follow up on test results and referrals
  • Implement and maintain patient care protocols and procedures

Qualifications



 

  • Proficiency in Electronic Health Record (EHR) systems and healthcare management software
  • Strong knowledge of medical terminology and healthcare procedures
  • Excellent organizational and time management skills
  • Outstanding verbal and written communication skills
  • Demonstrated ability to handle multiple priorities while maintaining attention to detail
  • Knowledge of healthcare regulations and compliance requirements
  • Experience in medical office administration
  • Strong problem-solving abilities and decision-making skills
  • Excellent interpersonal skills with a patient-centered approach
